Louis Domenico, MD is a board certified vascular and interventional radiologist with expertise in minimally invasive treatments for dialysis access management, varicose veins, uterine fibroids, peripheral arterial disease (PAD) and varicoceles.
Dr. Domenico is a graduate of Rosewell Park Cancer Institute Master’s Program and earned his medical degree from SUNY Buffalo Jacobs School of Medicine and Biomedical Sciences. He completed an internship in internal medicine at SUNY Buffalo Medical Consortium in Buffalo, New York. Dr. Domenico completed a residency in diagnostic radiology and a fellowship in interventional radiology at Albany Medical Center in Albany, New York.

David R. Hertzog, MD, FACS

David R. Hertzog, MD, FACS is a board certified general surgeon and interventional radiologist with more than 30 years extensive experience providing exceptional patient care. He specializes in dialysis access management for ESRD patients and the minimally invasive procedures which maintain proper function and reduce potential disruptions of scheduled dialysis treatments.
Dr. Hertzog earned his medical degree from the Medical College of Pennsylvania in Philadelphia, where he also completed his surgical residency. He worked as an attending general surgeon for nearly 20 years and then completed a fellowship in interventional radiology from Milton S. Hershey Medical Center in Hershey, PA.
In addition to academic appointments as Clinical Assistant Professor of Surgery and Clinical Assistant Professor of Radiology, Dr. Hertzog has served on many professional and scientific committees within the Capital Health System Medical Center, RWJ-University Hospital at Hamilton and St. Francis Medical Center. His work also includes several peer reviewed publications in the Journal of Vascular and Interventional Radiology, among others.
David R. Hertzog, MD, FACS is a member of the American Medical Association, Vascular Access Society of the Americas, Society of Interventional Radiology, and is a fellow of the American College of Surgeons.

Mark P. Mantell, MD is a board certified vascular surgeon specializing in dialysis access creation and revision. He treats patients in the greater Philadelphia area including those from Azura Vascular Care locations in Cherry Hill, Northeast Philadelphia, South Philadelphia, and Elkins Park.
Dr. Mantell has practiced in Philadelphia for over 27 years. He has been awarded Best of Philadelphia for many years by his peers. Mark P. Mantell, MD participated in access trials and coauthored multiple access related peer reviewed articles on surgical and endovascular approaches to access creation and maintenance and has a reputation as a regional expert in vascular access.
My father had to come to this center for an urgent blood clot removal from his dialysis AV graft. Setting this up was so swift, and because this was urgent I had many questions and the sweetest woman Kelly answered them all. They provided transportation to get my father all the way out in the suburbs into the city, and back to his local dialysis center in time for his treatment which went much better now that the clots were gone. He loved how he was treated and Azura and said the staff was amazing and very professional. Thank you all!
