Alexander S. Yevzlin, MD
Interventional Nephrologist
Medical Director
Alexander S. Yevzlin, MD is an interventional nephrologist specializing in dialysis access management. He is a past president of the American Society of Diagnostic and Interventional Nephrology (ASDIN), and is a pioneer in the subspecialty of interventional nephrology.
Dr. Yevzlin earned his medical degree from the Ohio State University College of Medicine and the Cleveland Clinic Foundation in Cleveland and Columbus, Ohio. He completed his internship and residency in internal medicine at the University of Michigan Hospital in Ann Arbor, Michigan. Dr. Yevzlin completed a fellowship in nephrology at the Northwestern University Memorial Hospital and Feinberg School of Medicine in Chicago, Illinois.
Dr. Yevzlin is a member of the American Society of Nephrology and the American Society of Diagnostic and Interventional Nephrology (ASDIN). Alexander S. Yevzlin, MD has co-authored numerous textbooks on interventional nephrology and dialysis access.

Robert B. Ballard, MD
Vascular Surgeon
Robert B. Ballard, MD is a board certified vascular surgeon specializing in minimally invasive treatments for a broad range of vascular conditions. He performs critical maintenance and management of vascular accesses for dialysis patients.
Dr. Ballard earned his medical degree from Michigan State University in East Lansing, Michigan. He completed his internship and residency at Temple University School of Medicine, Department of Surgery in Philadelphia, Pennsylvania. Robert B. Ballard, MD completed two fellowships at the Emory University School of Medicine, Grady Memorial Hospital in Atlanta, Georgia, and a fellowship in vascular surgery at Louisiana State University in New Orleans, Louisiana.

Aaron Schwaab, MD, FACS
General Surgeon
Aaron Schwaab, MD, FACS is a general surgeon with experience in dialysis access and the treatment of peripheral arterial disease and venous conditions, including minimally invasive procedures.
Aaron Schwaab, MD, FACS earned his medical degree from the University of Wisconsin Medical School and completed his general surgery residency at the University of New Mexico. He is a Fellow of the American College of Surgeons and is board certified by the American Board of Surgery.

I have been going to Azura surgery center in Madison since they put my fistula in and it is a wonderful facility. All the staff are friendly and answer all my questions and follow up with me after I have my fistula maintenance and I love that about Azura in Madison.

Very hospitable hospital. I felt everyone was concerned about me. But when they did the surgery all my family and friends were surprised they went ahead with the surgery and let me go with my blood pressure so high. But I hadn’t taken any medications in the morning which includes blood pressure medication. It came down after my nap when I got home. It was a good experience.
